For more than 40 years—first as CCNRC and now as Sagepoint—our community-focused nonprofit has provided our region with high-quality senior care. Today, we continue to grow in service, and offer an unmatched breadth of trusted care solutions.
Charles County Nursing and Rehabilitation Center is founded as a Not-For-Profit 501(c)(3) Corporation.The Center is governed by a voluntary Board of nine trustees appointed by the Charles County Commissioners. The Center initially can serve 105 patients.
The Adult Day Services Program is added. The program initially begins with 25 participants in the Nursing Center building.
The Adult Day Services Program building opens with 38 participants in the program.
The Nursing Center expands to serve 165 people and four units:
Addition to the Adult Day Services building is completed. It serves the day care dementia patients and others needing specialized care. Current licensure for the Program is for 63 participants.
Assisted Living comes to CCNRC; Abbey Manor is purchased from a Baltimore real estate firm. The purchase includes the property and two buildings of sixteen apartments, which gives CCNRC the ability to house 32 Assisted Living residents. The name “CCNRC Family of Care” is adopted.
The Commissioners of Charles County turns the properties and buildings over to CCNRC. The Board operates under bylaws developed and approved by the Board of Trustees; and is a self-sustaining Board.
Construction of a two-story medical arts building is completed on the main campus. The first floor is leased to Fresenius Medical Care, a dialysis provider. The second floor houses “Homeplace”, a 12-bed secure memory care assisted living community for adults with memory impairment/dementia. This community is a functioning household and uses state-of –the-art programming developed to assist the residents while enhancing and maintaining their quality of life.
The services offered by CCNRC are combined and re-branded under a single new name—Sagepoint Senior Living Services, offering:
Plans are announced for a new assisted living community, and Sagepoint Village. Sagepoint acquires Circle of Care and begins providing Home Care Services.
Phase I of new rehabilitation center opens. This beautiful wing boasts of 13 spacious private rooms, each with their own 40″ flatscreen, HD television. Rooms offer private bathrooms with European style showers. The new wing offers patients greater privacy and comfort!
Sagepoint breaks ground on Sagepoint Village, a 60 bed state-of-the-art assisted living community. The new building will be located at the intersection of Heritage Green Parkway and Washington Avenue in La Plata. The building will neighbor Sagepoint Gardens, Sagepoint’s existing 32-bed assisted living community. The new community is anticipated to be open in early 2019.
Sagepoint completes new rehabilitation center with 30 spacious private rooms, each with their own 40″ flatscreen, HD television. Rooms offer private bathrooms with European style showers. New rehabilitation center includes a larger gym with apartment style setting to include a fully working kitchen to make transitioning back home easier.
